Terms listed below in ALL CAPS correspond to the field names on the SERVICES screen of the FVC-APP.

PERSON SERVED
Select ONE from a Pull Down menu that contains all victims, alleged offenders and involved persons that have been entered for this incident.
Comment: Once a "PERSON SERVED" is selected, then entry of at least one service is REQUIRED.

There are three parts to the entry on this screen that tracks the services that were delivered.

I. Service Domain = NEED ADDRESSED
a. Choose one of 12 pre-defined SERVICE DOMAIN codes. Service Domains #1-10 are based on the Federal CBSG tracking categories.
b. The FBI recognizes 50+ offense categories, which will be used as a guide to develop the LUTs for service domains #11 and #12.
c. Note that the HEALTH Service Domain has 4 sub-domains.

II. Service = what you did globally.
a. See the draft list of Services below for definitions of each service.
b. Note that the definitions are fairly broad in order to facilitate tracking of similar services across agencies and across programs.
c. This initial list of services needs to be reviewed to make certain that it meets the basic reporting requirements of participating agencies – This review is in progress.

III. Unit of Service = how often/how much service you provided (AutoFills depending on  the SERVICE selected.)
a. A "unit" may be expressed in hours, 1 night, 1 one-way trip, 1 contact, 1 activity, etc. A specified service unit will be linked to each service, and the DET will merely have to record the number of units, e.g. ## bednights, ## home visits, etc.
b. In most cases the “PERSON SERVED” will be an individual. However, when calculating Units of Service it may be convenient at times (e.g. bednights, trips, etc.) to fill in the total Units of Service for additional members of the ‘Persons’s’ family.

SERVICE DOMAIN - REQUIRED when a Person Served is entered
Select the ONE service domain that is the BEST FIT for the NEED addressed through the provision of this service.
Comments: The first 10 service domains are based on the "need addressed" domains used for reporting on activites funded through [Federal] Community Service Block Grants (CSBGs). Service domains 11 and 12 are specific to the NEED for protection from abuse and neglect and the NEED for law enforcement.

SERVICE PROVIDED - REQUIRED when a Person Served is entered
Each of the 12 Service Domains will have a separate Pull Down menu with specific categories of service that address the NEED area for that Service Domain. The Data Entry Technician will list each service provided under the most relevant service domain.
Special Instructions: The list of Services Provided is under development, and attempts to categorize the range of possible services provided in broad general categories. This is NOT a case management database, so the services are described globally, rather than being specific to individual programs. The goal for reports generated from the interagency FVC database is to aggregate the types of services that each type of 'involved person' served received and to look at patterns of service delivery.

The proposed list of Services is displayed below. The Service Domains that apply to each SERVICE are in parentheses, followed by the Service Description and the Unit of Service. For some services (e.g. a Referral) it will be necessary to include an Organization and Sub-Unit.

SERVICE UNIT - Auto fills when a Service is selected
When a 'Service Provided' is selected a unit of service will auto fill the field below.
Special Instruction: When there are multiple members of a family, it may be convenient to select the 'head of the household' as the person served and then just fill in the number of units (e.g. bed nights, one-way trips, etc.) that the entire family received.
NUMBER OF UNITS - REQUIRED when a Service Provided is entered
Fill in the correct number of Service Units.
 
RECOMMENDATION: In order to accurately aggregate data on services provided, addition of a field that will allow the data entry technician to respond to the question: "Was this service was provided to multiple members of a household?" (Yes/No) is desired.

Other Organization
The Data Entry Technician will enter another organization, as indicated using the exising organization and organization sub-unit Pull Down lists. For example for the Service "Networking" you would enter the name of the Organization and Sub-Unit that you contacted on behalf of the client.

Other Org Sub-Unit
The Data Entry Technician will select the Organizational Sub-Unit that was contacted from the Pull Down list of Sub-Units (programs) given for each Organization.

Service Begin Date
The Data Entry Technician will enter the date that the service was initiated and/or delivered. This field is not required.

Enter Another Service Record
The Data Entry Technician can toggle back and forth between Services Provided records using the PREVIOUS RECORD and NEXT RECORD buttons.

Send all data for this incident to the FVC Database
This button sends the record for the entire incident to the FVC Database. The Garrett Data Mart can then link this record with records from other databases  through the use of the Client Identification Hashing Module or Hash ID. A safety message will also appear on the screen asking if you want to send the record to the database.
Comment: Prior to sending the entire record to the FVC Database, the Data Entry Technician (DET) can toggle back and forth between the various TABS to correct errors/update information. However, once the record is sent to the database, the DET CAN NOT retrieve the record. If an organization wishes to keep track of data that they have entered, the organization can ask their DET to print out any or all of the screens prior to sending the incident record to the database.
